Personal Action Towards Health (PATH)

Two trained facilitators lead a six-week workshop that gives participants the skills and strategies needed to better manage chronic health conditions (such as arthritis, asthmas, COPD, or depression). The workshop meets once a week for 2 ½ each week at different locations within the community.
