MI Choice Waiver

MI Choice Waiver Program is a home and community-based care option for persons who are Medicaid Waiver eligible for their long-term care. It is less costly and a preferred alternative to nursing home care, ensuring people get the support they need in their own homes.

MI Choice is funded by the Department of Community Health and Human Services (MDHHS). This program provides a wide range of home and community-based services coordinated by a care manager to ensure participants are able to remain as independent, healthy, and safe as possible within their own community.

You may qualify if:

  • You are a senior age 65 or older
  • You are a disabled person age 18 or older
  • You live in Detroit, the five Grosse Pointes, Hamtramck, Harper Woods, and Highland Park
  • Your income does not exceed $2,742 a month and your personal assets do not exceed $2,000 a month, excluding your residence and one vehicle
  • You have multiple medical and service needs
  • You need the waiver program to prevent having to move into a nursing home

This program may provide some of the following services:

  • Community transition services
  • Community living supports
  • Nursing services (preventative nursing)
  • Respite services
  • Adult day health (adult daycare)
  • Environmental modifications
  • Non-medical transportation
  • Medical supplies and equipment not covered under the Medicaid State Plan
  • Chore services

  • Personal emergency response systems
  • Private duty nursing
  • Counseling
  • Home delivered meals
  • Training in a variety of independent living skills
  • Supports coordination
  • Fiscal intermediary
  • Goods and services

MI Health Link

The MI Health Link program is administered by the health plans that provide care coordination and Long-Term Support Services for the dual-eligible (Medicare and Medicaid) members.

The Detroit Area Agency on Aging is a provider for the health plans for residents of Detroit, the five Grosse Pointes, Hamtramck, Harper Woods, and Highland Park.
