MI Choice Waiver

MI Choice Waiver Program is a home and community-based care option for persons who are Medicaid Waiver eligible for their long-term care. It is a less costly and preferred alternative to nursing home care that allows people to get the support they need in their own homes.

MI Choice is funded by the Michigan Department of Community Health and Human Services (MDHHS). This program provides a wide range of home and community-based services coordinated by a care manager to ensure participants are able to remain as independent, healthy and safe as possible within their own community.

You may qualify:

  • If you are a senior age 65 or older
  • If you are a disabled person age 18 or older
  • If you live in Detroit, the five Grosse Pointes, Hamtramck, Harper Woods and Highland Park
  • If your income does not exceed $2,382 a month and your personal assets do not exceed $2,000 a month, excluding your residence and one vehicle
  • If you have multiple medical and service needs
  • If you do not get assistance you will have to move into a nursing home

This program may provide some of the following services:

  • Community transition services
  • Community living supports
  • Nursing services (preventative nursing)
  • Respite services
  • Adult day health (adult day care)
  • Environmental modifications
  • Non-medical transportation
  • Medical supplies and equipment not covered under the Medicaid State Plan
  • Chore services
  • Personal emergency response systems
  • Private duty nursing
  • Counseling
  • Home delivered meals
  • Training in a variety of independent living skills
  • Supports coordination
  • Fiscal intermediary
  • Goods and services
