The Detroit Area Agency on Aging (DAAA) is proud to introduce its Home-Based Primary Care (HBPC) Program, a transformative new initiative to meet the healthcare needs of adults who are homebound or face significant barriers accessing traditional medical services.
Home-Based Primary Care will serve communities across Wayne, Macomb, and Oakland counties, becoming the only Area Agency on Aging-supported primary care initiative of its kind in Southeast Michigan.

· Comprehensive Assessment: We begin with a thorough evaluation of your health needs to tailor a care plan that works for you.
· Monthly Home Visits: Our healthcare professionals provide consistent, in-home checkups to monitor your well-being and address concerns early.
· Medication Management: We ensure your prescriptions are up to date, effective, and safely managed.
· 24/7 Provider Access: Just like all primary care offices, a healthcare provider can be reached any time, day or night- because we know your health can’t wait.
· A Long History of Service: From Meals on Wheels to Telehealth services, our commitment to providing accessible care that has improved quality of life and allowed our community to age with dignity has spanned over 40 years.
· Access to Medical Specialists In: Behavioral Health, Pain Management, Physical and Respiratory Therapy, and Chronic Disease Prevention & Management.
· In-home instruction in evidence-based programing, including Fall Prevention and Pre-Diabetes/Diabetes counseling and education
Post-Hospital Support:
If you’re hospitalized for more than three days, HBPC steps in to ease your transition home:
· Home Visit Within 48 Hours: An RN visits you shortly after discharge to review medications and support your recovery.
· Ongoing Recovery Support: For three months post-discharge, we stay connected through scheduled calls and home visits to ensure continuity of care and peace of mind.
Who Is Eligible
Our Home-Based Primary Care Program is designed primarily for insured individuals who are homebound or have difficulty traveling to medical appointments. However, the program will also accept those who may benefit from ongoing in-home primary care services.

Home-Based Primary Care is more than convenience, it’s a proactive, evidence-informed approach to reducing avoidable hospitalizations and emergency room visits.
