NCQA Accreditation

We're proud to announce that we've received 3 Year Accreditation from National Committee for Quality Assurance (NCQA) in Case Management for Long-Term Services and Supports (CM-LTSS)! 
 
NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. NCQA’s Web site, www.ncqa.org, contains information to help consumers, employers and others make more informed health care choices.
 
NCQA’s Accreditation of Case Management for LTSS program is designed for home and community-based organizations, health plans that only provide LTSS benefits and other organizations that coordinate LTSS. It can be used to align goals and approaches to LTSS across payers and providers. This is a voluntary review process with high standards to encourage organizations coordinating CM-LTSS to achieve a high level of performance, increase adherence to care guidelines and create an environment of continuous improvement. 
 
NCQA Accreditation standards are developed with input from researchers in the field, the LTSS Advisory Committee and standing committees, both purchasers and operators of LTSS programs, state and federal regulators and other experts. The standards include eight areas which include Program Description; Assessment Process; Person-Centered Care Planning and Monitoring; Care Transitions; Measurement and Quality Improvement; Staffing, Training and Verification; Rights and Responsibilities; and Delegation.
