Take Heart: Heart Health Program

This is a heart health education program designed for adults 50 years of age and older to learn to manage heart disease and related risk factors. It is a randomized clinical trial/research study by the University of Michigan (U-M), in partnership with the Detroit Medical Center (DMC), Wayne State University Physician Group and Detroit Area Agency on Aging (DAAA). Potential participants must complete a phone survey with U-M to determine eligibility for the program. The workshop meets for seven weeks and consists of five group sessions that last 2 ½ hours and two phone calls with the Health Educator.
