Project Choice

The Project Choice/Care Management Program funded by the Aging & Adult Services Administration (AASA) helps families find, mobilize and coordinate a variety of homecare and other services needed by frail, homebound older adults at risk of entering a nursing home or other institution. The program promotes independent living at home. Certain criteria must be met to be considered for enrollment in Project Choice.

You may be eligible if:

  • You are a frail or disabled individual age 60 or older
  • You are medically eligible for nursing home placement, but financially ineligible for the Medicaid Waiver Program
  • You meet the medical eligibility for nursing facility level of care
  • You are currently living in a facility, however you no longer need all of the services provided there, but will need help when you return home

The Project Choice Program may provide some of the following services:

  • Chore Services
  • Home Delivered Meals
  • Adult Day Care
  • Home Care Assistance
  • Transportation
  • Caregiver Respite
